First Aid at Christ Church Purley
First aid boxes can be found – in the Foyer kitchen and each large room / kitchen.
Please complete accident form – use the QR code below for ALL accidents or alternatively there are accident books where paper copies can be completed – find these by the first aid boxes.


Please let Jean know if a paper form has been completed as we need to keep these securely in the office to comply with data protection.

If you spot items are depleted in any of the boxes please let Jean know in the office ASAP
